How To Create An Account

You’ll first need to create an account in order to start leaving reviews, managing your listing, or connecting with providers. You can do this by following the steps below.

1. Start by clicking the on the top right of the page
2. Fill out the information in the attached sheet, you can also “Sign up with Google” to make signing in easier. All information is required.
3. Once you’ve submitted that, fill out the profile information.
4. Important: Make sure to click the right “User Type” as a user/patient cannot claim a practice.
5. Once this information is submitted you will now be able to login to your account using the email and password you assigned!
